Find Facilities searches the national catalog by name, city, or ZIP, and can narrow by facility type and state. A result is a CMS-reported record. It can show facility type, location, whether the facility is in the latest successful CMS snapshot or only an earlier one, and the source the fact came from. For hospitals, classification is shown as reported by CMS. These are the facts in the indexed sources. They are not a complete account of the facility.

Each facility profile opens with a Professional snapshot of facts already in the CareRecon record. When CMS reported them, that snapshot can include the CMS identifier, phone, address, ownership type, and the snapshot date. Hospitals can include classification and the emergency-services value CMS reported. Nursing homes can include the date CMS reported as first approval for Medicare and Medicaid services. Home health, hospice, and inpatient rehabilitation records can include the certification date CMS reported. A home health or hospice address is an office location, not the provider’s service area.

Unknown is not no. A field that was not in the source is omitted. Missing is not absent. A gap is not proof that a service, capability, or condition is absent. CareRecon does not infer clinical capabilities from a name, a brand, a city, or a health-system affiliation.

Know what still needs verification

Before accepting a contract, making a referral, or citing a record, a professional still has to verify anything CareRecon does not establish: real-time bed or admission availability, payer or network acceptance, whether a facility is accepting referrals, clinical suitability, and operating status beyond the CMS snapshot.

Nursing-home profiles can show CMS-reported staffing hours, turnover figures, inspection dates, deficiencies, and penalties when those CMS files have been indexed for that facility. Other facility types do not have those fields in this catalog. CareRecon would not turn hours per resident per day into a nurse-to-patient ratio, infer private equity from a company name, treat a patient survey as an employee review, or produce a workplace score. A gap is not a finding about this facility.
