The service
CareRecon is a product of Elevar Resilience LLC.
CareRecon is an informational directory of public healthcare facility records. You can look up a facility and read what the indexed source reported. The current records cover hospitals, nursing homes and skilled nursing facilities, home health agencies, hospice providers, and inpatient rehabilitation facilities.
No medical advice
CareRecon publishes public facility information. It does not provide medical advice, diagnoses, or treatment recommendations, and it does not tell you which facility to choose.
Emergencies
If you think you have a medical emergency, call your local emergency number. In the United States, that number is 911. Do not use this site for an emergency.
No recommendation or ranking
Results are ordered by the facility name on the record. That order is not a quality ranking and not a recommendation. The site does not publish star ratings or “best facility” scores. Other facilities shown from the same state and the same facility type are further records from that directory. They are not a recommendation and not a distance ranking.
Advertising, if shown, is labeled and kept apart from facility facts. It does not change search order, rankings, or how records are interpreted.
Public sources
The records come from public datasets of the Centers for Medicare & Medicaid Services (CMS). CareRecon is independent of CMS. This is not a government website. CMS does not operate CareRecon, and a listing is not a CMS endorsement of CareRecon or of the facility.
Each profile states the source it was drawn from. “Last verified” means when CareRecon last included that facility in a successful CMS import. It is not a promise that every detail was confirmed that day, and it is not a CMS endorsement. How to read a record is explained on About the data.
Data can change
CMS datasets change. A later import can add a record, change a record, or show that a facility was not in the latest successful import. When a facility was not in that import, the page keeps the earlier CMS record and says so. Absence from the current file is not a statement about whether the facility is operating.
The site can be unavailable. These terms do not promise that the site will be reachable without interruption.
Unknown and missing information
Unknown is not no. If a field was not in the indexed source, the site omits it. Omission is not proof that a service, capability, or condition is absent. The site does not fill gaps by inferring clinical capabilities from a name, a brand, a city, or a health-system affiliation.
Accuracy
The site shows the facts present in the indexed source, together with the provenance on the record. It does not guarantee that a record is complete, current, or free of source error beyond that provenance.
Acceptable use
Use the site to read facility information. Do not send medical records or someone else’s health information to the site. Do not try to disrupt the site, break into it, or bypass ordinary access. Do not use automated requests in a way that degrades the site for other people.
Ordinary reading of public pages is allowed. These terms do not give you a separate license to republish CMS datasets. Those datasets stay subject to their own public terms.
Third-party services
The Privacy page describes hosting, the facility database, and optional analytics. Facility pages show CMS fields on this site. They do not send you to another website for those facts. If advertising is configured later, it is labeled and kept apart from facility facts.
Accounts and submissions
This site does not offer accounts, reviews, or a way for the public to submit facility corrections or community reports.
Limits
You are responsible for how you use what you read. The site is not a substitute for a clinician, a facility, or emergency services. It is not responsible for decisions you make from a listing. Availability and accuracy are limited to what this page and the provenance on each record describe.
