What this site is
CareRecon is a product of Elevar Resilience LLC.
CareRecon publishes public healthcare facility records from the Centers for Medicare & Medicaid Services (CMS). The current records cover hospitals, nursing homes and skilled nursing facilities, home health agencies, hospice providers, and inpatient rehabilitation facilities.
CareRecon publishes public facility information. It does not provide medical advice, diagnoses, or treatment recommendations, and it does not tell you which facility to choose.
Accounts and health information about you
There are no user accounts, patient accounts, or clinician accounts. The site does not ask you to sign in. It does not collect medical records, diagnoses, insurance membership, or other health information about you in order to show the directory. The records on the site are about healthcare facilities, drawn from public CMS sources.
Do not send medical records or someone else’s health information to this site. There is no account or form here for that information.
Facility search
Facility search uses a GET request. The page address can include the words you typed and filters such as facility type, state, city, ZIP, and page. You can copy or share that address. A hosting provider that logs requests may record the address, including the search text. The application server also uses the search text to query the facility database. Product analytics is separate and is described below.
Choices saved in this browser
This application does not set a cookie to remember your privacy choices. It stores them in local storage in this browser. The stored choice records whether analytics is on, whether advertising is on, and the time you saved it. That save happens when you choose “Keep optional tools off” or “Save choices.” You can reopen the choice with “Privacy choices” in the footer.
The directory can be read without saving a choice. Until you save one, analytics and advertising stay off. Clearing this site’s data in your browser removes the saved choice.
Analytics
Analytics stays off until you opt in. If you opt in, the site loads Vercel Web Analytics. Custom events are limited to: a search was submitted, a facility category was selected, a state was selected, a facility profile was opened, and pagination was used. A custom event may include a facility-type identifier, a two-letter state code, or a page number. It is not built to include the text you typed, a facility name, or health information about you.
Analytics page views have the query string removed, so search text in the address is not part of that page view. The page path can still show which facility, category, or state page was open.
Advertising
Advertising stays off unless you opt in. Opting in stores that choice and does not load an advertising script. A labeled advertising area is rendered only when advertising has also been turned on for the site. The application does not load a third-party advertising script. While advertising is off, no advertising area is shown.
If an advertising area is shown later, it stays apart from facility facts. It does not change search order or how a record is read. See the Terms.
Who processes information for this site
Vercel hosts the website. A request for a page, including a search address, may be recorded in the host’s request logs. This page does not state how long those logs are kept.
Supabase hosts the PostgreSQL database of facility records. The website server queries that database to answer a search or open a profile. Your browser does not query those application tables through a public data API.
Vercel Web Analytics receives the analytics described above only after you opt in.
CMS is the source of the published facility records. CMS does not operate this website.
